Terms and Conditions for CredHawk

Welcome to CredHawk. These Terms and Conditions ("Terms") govern your access to and use of the CredHawk mobile application, website, and related services (collectively, the "Service") provided by AlterMed RCM LLC ("AlterMed RCM LLC," "Company," "we," "us," or "our").

By downloading, accessing, or using the Service, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree to these Terms, you must not access or use the Service.

1. Description of Service

CredHawk is a business-to-business (B2B) provider management platform designed to help healthcare organizations credential, onboard, enroll, and manage licensed healthcare professionals across multiple healthcare sectors.

The Service provides workflow tools for managing provider information, credentialing, licensing, enrollment, compliance, document management, reporting, and related administrative activities.

CredHawk supports healthcare organizations including, but not limited to:

  • Physician practices
  • Hospitals and health systems
  • Dental organizations
  • Behavioral health organizations
  • Therapy organizations
  • DME suppliers
  • Home health agencies
  • Other healthcare provider organizations

2. Eligibility and User Accounts

Eligibility

The Service is intended only for individuals who:

  • Are 18 years of age or older;
  • Are authorized to act on behalf of a healthcare organization or other authorized business entity; and
  • Have been granted permission to access the Service by their organization.
Account Security

You are responsible for maintaining the confidentiality of your login credentials and for all activities occurring under your account.

You agree to notify AlterMed RCM LLC immediately of any unauthorized use of your account or any security breach.

Accuracy of Information

You agree that all information submitted through the Service is accurate, complete, and kept up to date.

3. Data Privacy, Security, and HIPAA Compliance

Where applicable, AlterMed RCM LLC handles information in accordance with applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

If required, use of the Service may also be governed by a separate Business Associate Agreement (BAA) between AlterMed RCM LLC and your organization.

Our collection and use of personal information are governed by our Privacy Policy.

4. Device, Connectivity, and Service Availability

Access to the Service requires:

  • A compatible mobile device or supported web browser
  • Internet connectivity or mobile data service
  • Supported operating system versions

You are responsible for any internet, mobile carrier, or data charges incurred while using the Service.

Certain features, functionality, or services may not be available on all devices, operating systems, geographic locations, or subscription plans. AlterMed RCM LLC reserves the right to modify, suspend, or discontinue features at any time without prior notice.

5. Acceptable Use

You agree not to:

  • Violate any applicable local, state, federal, or international laws.
  • Upload malicious software, viruses, or harmful code.
  • Attempt unauthorized access to the Service or other user accounts.
  • Interfere with the operation or security of the Service.
  • Infringe upon intellectual property or privacy rights.
  • Use the Service for unlawful or unauthorized purposes.

6. Credentialing and Workflow Disclaimer

CredHawk provides workflow management, provider management, credentialing, licensing, enrollment, compliance, and document management tools.

Use of the Service does not guarantee:

  • Credentialing approval
  • Licensing approval
  • Payer enrollment approval
  • Insurance network participation
  • Provider reimbursement
  • Employment or contracting decisions
  • Regulatory compliance determinations

All approval decisions are made solely by licensing boards, government agencies, payers, healthcare organizations, employers, or other authorized third parties. Users remain responsible for ensuring the accuracy and completeness of submitted information and complying with all applicable legal and regulatory requirements.

7. Intellectual Property

All software, content, trademarks, logos, graphics, documentation, text, user interface designs, workflows, and other materials contained within the Service are owned by AlterMed RCM LLC or its licensors and are protected under applicable intellectual property laws.

You are granted a limited, non-exclusive, non-transferable, revocable license to use the Service solely for your organization's internal business purposes.

8. Disclaimer of Warranties

THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE."

ALTERMED RCM LLC MAKES NO WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, OR THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, COMPLETELY SECURE, OR FREE FROM DEFECTS.

9. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, ALTERMED RCM LLC, ITS AFFILIATES, OFFICERS, EMPLOYEES, CONTRACTORS, OR AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, OR PUNITIVE DAMAGES, INCLUDING LOSS OF PROFITS, DATA, BUSINESS INTERRUPTION, OR GOODWILL ARISING FROM THE USE OF OR INABILITY TO USE THE SERVICE.

10. Termination

AlterMed RCM LLC may suspend or terminate your access to the Service at any time, with or without notice, if you violate these Terms or if continued access presents a security, legal, or operational risk.

Upon termination, your right to access and use the Service immediately ends.

11. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the state in which AlterMed RCM LLC is headquartered, without regard to conflict of law principles.

Any legal action arising from these Terms shall be brought exclusively in the appropriate state or federal courts located within that jurisdiction.

12. Changes to These Terms

AlterMed RCM LLC reserves the right to modify these Terms at any time.

Material changes may be communicated through the application, website, or email. Continued use of the Service following the effective date of revised Terms constitutes acceptance of those changes.
